New Pilot joining forms (Copy complete and send all four)

1. This form requires all of your contact details to become a member of SAC and receive a membership number.

2. This form requires similar details but requires a tick in the Novice box and details of any previous flying experience.
|
|
3. This form needs to be filled out by your own doctor or GP after a medical checkup to the driver licence standard.
|
|
4. This form is a declaration of fit and proper person status which must be filled out, signed and sent with the other forms.
Joining fee for a new member is $100 Send the forms to [email protected] or SAC, PO Box 10324, Te Rapa, Hamilton 3288. Direct Banking Details are: National Bank Account #06-0622-0028104-00. Please identify yourself clearly in the particulars/reference fields. |

This form is only required by existing members who need to renew the SAC subscription. (Cost $95)
